Practices in the UK have used paper forms to gather patient information, requiring reception to manually scan the forms into Ascend, and then check each form manually, make manual updates (such as changes to demographics like address & phone) and create Medical Alerts as required.
Now with the Patient Forms feature release, this allows practices to send the following forms electronically to patients to fill out prior to their appointment - saving both time and effort for both the patient & the practice staff. This includes the following forms:
- Terms of use
- Demographics Form
- Oral Health Survey
- Medical History Form
- Custom TX Plan Consent Messages
Enabling Patient Forms
Patient Forms can be enabled from the Patient Forms menu (Under Settings). Each Form can then also be toggled between Active & Inactive as required.
Sending Patient Forms
There are three main area's where you will be able to view and request patient forms:
- The Patient Overview Screen
- The Patient Forms tab under the Patient Information menu
- Forms can also be requested from the patients' charting screen
When clicking the Request Forms OR Due Now button you will be able to send the patient form via one of three methods:
- Via text message
- Assuming the patient has a valid mobile phone number
- Via Email
- Assuming the patient has a valid email address
- Via Copy Link which will copy the Web Address so it can be provide to the patient outside of Ascend if required
Reviewing Patient Forms
Once the patient completes the form and the changes have been saved, the form will automatically appear within Patient Forms (under Patient Information Settings) as well as in the Document Manager window
Patient Demographics Form Write Back
When enabled, any updates a patient makes to the Demographics form will automatically be reflected within Ascend
- This means no manual updates of patient information required to be completed by the practice staff
- This can be enabled from Settings --> Patient Forms --> Demographics --> Update patient details --> ON
