You can provide a printed Personal Dental Treatment Plan (NHS form FP17DC) to a patient. The form contains the details of proposed NHS and private treatment and the associated fees. In addition to providing the details of the case, the form provides important information regarding how to use the NHS dental services. Once the patient has looked over the treatment plan and estimate, if he or she wants to have the recommended treatment performed, he or she can sign the form to indicate acceptance of the NHS and private treatment and of the associated fees.

How to get there
Use the Patient Search box to access a patient's record.
Notes:
If the last patient record you accessed since you last logged on is the correct patient's record, you do not need to re-enter the patient's name in the Patient Search box.
You can include inactive patients in the search results by setting the Include inactive patients switch to On.
Do one of the following:
Skip step 1.
On the Patient menu, under Clinical, click (or tap) Treatment Planner.
The patient's clinical record opens with the Tx Planner tab selected.
On the Tx Planner tab of a patient's clinical record, under Active, select a case.
The recommended treatment for the selected case appears.
Click (or tap) FP17 DC Estimate.
The NHS Dental Care page appears on a new tab of your Web browser.
Click (or tap) Print.
