The FP17PR form is a legal document that's part of a patient's dental records. Ascend makes it easy for the patient to fill in the FP17PR form online prior to their appointment, via their patient portal and for you to keep the completed form online in the document manager.
The ethnicity and exemption details they've filled in will automatically be added to the associated claim, making it quick and easy to get the claim filled in.
The form will also be stored in the document manager, easily accessible if you need to check the patient's details.
Enabling the FP17PR Form
The Patient Forms functionality needs to be enabled in order to use the FP17PR Form.

Requesting the FP17PR Form
To send your patient the FP17PR form, use the 'Request Forms' link located on the patient's Overview page, or in their Patient Information section.

Completing the FP17PR Form
The FP17PR form has important default information filled out, making it quick and easy for your patients to review, complete and sign.

Attaching the FP17PR Form to Claims
You can attach the FP17PR form to the patient's claim, from within the claim window, under the FP17PR Form label.
Note: For simplicity, if you have one claim open for a patient and one FP17PR form, this will be selected by default. If multiple PR Forms are available, then you can review and select the correct form for the claim.
The ethnicity data, exemption details and supporting details are all automatically updated on the claim based on the FP17PR Form selected -
If required, you can review the FP17PR form from the patient's Document Manager or from the patient's Overview page.
Note: The date recorded on the stored form uses the US date format
