Previewing patient form templates

You can quickly see what kind of information patients will enter on a given questionnaire.

To preview a patient form template

How to get there

1. If you are not already viewing the correct location, select it on the Location menu.

2. On the Settings menu, under Location, click (or tap) Patient Forms.

   The Patient Forms page opens.

1. On a location's Patient Forms page, under Questionnaires, select a patient form.

   The options for the form become available.

   

2. Scroll the Example value box as needed to see all the questions on the form.
