To activate a patient form
How to get there
If you are not already viewing the correct location, select it on the Location menu.
On the Settings menu, under Location, click (or tap) Patient Forms.
The Patient Forms page opens.
On a location's Patient Forms page, under Questionnaires or Consent Forms, select an inactive form (the name will be in grey text to indicate that it is inactive).
The options for the form become available.
Set the Active switch to On.
From the Form Expiration list, select.
If you are activating the Demographics questionnaire, to have Dentrix Ascend automatically update a patient's record with information on a Demographics questionnaire that the patient completes online, set the Update patient details switch to On. With the switch set to Off, you must manually compare a completed Demographics questionnaire with what is in the patient's record and make any needed changes.
Click (or tap) Save.
